Avoiding problems with your new drug card

At the Drug Store: Present your new DC 37 Innoviant ID card to your local Innoviant Network pharmacy. If you do not have the card, call the DC 37 Health and Security Plan at 1-212-815-1234.

Mail Order Program: You must first register with InnoviantRx. You can

• Register online through the website at www.innoviantrx.com, or
• Complete the InnoviantRx mail order envelope with information for you and dependents, or
• Call InnoviantRx to register by phone at 1-877-390-9200.

If you have a new prescription to fill: Send the prescription to InnoviantRx in a completed InnoviantRx mail order envelope.

If you have re-fills outstanding from ESI and have already registered with InnoviantRx, have your re-fill Rx numbers available to order refills on line at www.innoviantrx.com, to use the mail order envelope (enclosing payment) or to order by phone at 1-877-390-9200.

Retail 90 Rx Program: To fill a 90-day prescription at retail you will need a new 90-day prescription with 3 (90 day) refills. You will pay three co-payments for a 90 day supply, compared with 2 co-payments by mail order.
